I haven't tried that one, but I know there are a couple of others that only work if you're at war with the civ whose planet you're targeting. Are you at war with this one?

It's a bug from the base version of the game on Epic, unfixed in Supernova. If you rotate between your planets using the left and right arrows some of the buildings disappear. As a work around, if you exit out of the planet window, and directly bring up the planet window they will show correctly.
I can't answer #2, but for #1, I've been playing all the way through the beta and have never seen a citizen change ideology. As far as I know it's set when the citizen appears and never changes.

#2 has been reported multiple times dating back to the beta of the base game on Epic.
I think perhaps Ramma is talking about the battle report in the GNN window on the left, and Jaia is talking about the triangular icon on the galaxy map in the spot where the battle took place. I also don't see a way to remove that one, although it does go away after a few turns.
@Illauna on Discord told me he had gotten the achievement for The Everwar challenge, so I've moved it to the working list.
@Wonderdeer on the GC4 Discord informed me that he had managed to get the achievement for Play a Multiplayer Game, so I've added that to the working list in the original post.
I just won a prestige victory as the Navigators and the achievement was not awarded, so I've moved that achievement to the broken list.

Some races have Factions that say they provide relic bonuses too, like the Navigator's Precursor Heirs faction. I'm not sure those work either.
Ships repair slightly faster at shipyards and slightly faster still at starbases. There's also icon to the right of the ship/fleet window in the lower left (when the ship/fleet is selected) that you can click on to speed up repair also, that one puts the ship/fleet to sleep until you cancel it or it is fully repaired.
Unused production from the previous turn is applied to the current project. That's probably what you're seeing.
Old-Spider reported that he was able to get the Rebuild the Retribution achievement, apparently it was fixed in 2.01, so I've updated the original post to put it in the "working" list.
I finally tried the Yor again for the first time since GC3 and got the Win as the Yor, Yor Dead Iconians (defeat the Iconians as the Yor), and Artifact Mastery achievements, so I've edited the list in the original post to move them to the working list.
Open up the game. At the main menu you'll see this: That will open this window: Click on the ship you want to download and you'll see this:
They should repair at 1 point per turn in open space, slightly faster if docked at starbases/shipyards/planets. What race are you playing as? There's at least one ability or trait that removes ship repair.
I only have the base game on Epic, not Supernova, but Epic shows I have 34/72 achievements.
Don't know if this will help, but I'm told that it's cheaper to retrofit when your ships are closer to a shipyard.
Old Spider reported on Discord that after 795 hours of play he finally got the achievement for playing 5 hours. The thing is, to get it, he had to play 5 hours continuously. This achievement was reported as fixed in one of the change logs but it's obviously still not working as intended. If the 100 hour achievement is coded the same way it'll be nearly impossible to get.

In the base GC4 on Epic you couldn't get more than one type of each ship upgrade. If you had one in the queue and didn't use it before discovering another of the same type you'd lose the new one. I wasn't aware that this had been changed for Supernova. If it has been I'd certainly support scifi1950's suggestion.
